This contract is mutually agreed upon by ___________________ also known as “Client” and _________________, “ also known as “DJ”, on _________________ (date of contract).
SERVICES PROVIDED
The Client hereby engages the DJ to provide a DJ Service. The service is to be performed within the following guidelines:

If event is held outside, proper cover and power must be provided by Client, they are necessary, for DJ equipment and operation of equipment.
SERVICE DETAILS
The said DJ Service shall consist primarily of providing musical entertainment by means of a recorded music format.
The DJ Service shall be conducted at the time and place listed above. Extended times may be available for additional fees as outlined in the fees section (4c) and must be agreed upon prior to the end of the scheduled event. The contracted venue may be changed with ample notice. No additional fees will be required if new venue is within ___ miles of old venue and DJ is given more than one-week (5 business days) notice.
The DJ hereby agrees to render his professional services and is at all times to have complete control of his program and equipment.

DJ will have backup equipment at the event.
DJ requires approximately one hour for setup and will be ready to provide music and/or entertainment at least fifteen (15) minutes prior to start of Client’s event provided, event place is open and/or accessible to DJ. DJ requires a minimum of thirty (30) minutes for break down of equipment.
DJ will at all times have at least one DJ/Entertainer in the DJ area to provide assistance and ensure proper function of the DJ equipment.

FEES
A deposit of $150 is required at signing of Contract.
Deposit is “Exchange Only” (Another Day) if cancelled within 30 days of event.
Standard packages are as follows:
DJ Music $1_____ per hour (minimum three hours)
Three hours of Music only – $_____________
Extended or additional time will be provided in half-hour increment and vary depending on type, location and hours of event, but will not exceed $50 per ½ hour.
